Preservation rhinoplasty (also called dorsal preservation rhinoplasty) is a modern technique that reshapes the nasal bridge by repositioning existing bone and cartilage rather than removing it. The natural dorsal aesthetic lines are preserved and lowered as a unit. This produces smoother contours and a more natural appearance compared to classical structural rhinoplasty.

Structural rhinoplasty removes the dorsal hump and rebuilds the bridge using cartilage grafts and osteotomies. Preservation rhinoplasty keeps the dorsum intact and lowers it from below, preserving the natural aesthetic lines, ligaments and skin envelope. Preservation typically gives smoother results with no risk of an open-roof deformity.

Most primary rhinoplasty patients with a smooth or moderate dorsal hump are excellent candidates. Even some crooked noses and asymmetric dorsums can be addressed with preservation principles. Dr. Erdal evaluates each case individually through detailed photo analysis to determine the most suitable preservation approach (let-down, push-down, low septal resection or high septal resection).

Both push-down and let-down lower the bony pyramid as a single unit. In push-down, the pyramid is impacted into the maxilla. In let-down, a precise wedge of bone is removed from the lateral nasal wall so the pyramid descends in a more controlled way. Let-down is generally preferred for larger humps and offers more reliable lowering.

These describe where the septal cartilage is divided to allow the dorsum to descend. In LSR, the cut is made low along the septum near the nasal floor — preferred for many primary cases. In HSR, the cut is made high under the dorsum — useful in selected cases. Dr. Erdal has published peer-reviewed papers on both LSR and HSR techniques performed via the closed approach.

The nose looks significantly refined within 2–3 months as major swelling resolves. Most patients have a well-established result at 1 year. Those with thicker skin may see gradual refinement continuing over 2–3 years.

Generally, yes. Because the natural dorsal aesthetic lines, the underlying ligaments (such as Pitanguy's midline ligament), and the skin envelope are preserved rather than disrupted, the result tends to look unoperated. There is also no risk of an open-roof deformity since the dorsum remains intact as a single unit.
